| This traditional
funeral package includes the minimum service and overhead fee and the following: |
| • | Vehicle and equipment for transfer of remains
to funeral home from place of death (within a 40-mile radius) | | • | Embalming
or alternative preparation of the remains in accordance with Oregon State Law |
| • | Dressing, cosmetics, and casketing | | • | Use
of our facilities and staff for a viewing | | • | Use
of our staff at our chapel or another facility for a memorial service |
| • | Committal services at a local cemetery (within a 40-mile
radius) | | • | Funeral coach and service vehicle/flower
van | | • | Printed matter package consisting of a register
book, memorial folders, and acknowledgment cards |

and are subject to change without notice.
For your convenience, we offer
several funeral packages. The package price offered here is less than the sum
of the itemized portion. If any items contained in the package are not used or
desired, there will be no credit or deduction from the price of the package.
Packages do not include cemetery charges, casket or alternative container,
outer burial container, or cash advances. Death certificates are available in
addition to the package at $20.00 per copy for the first copy, and $15.00 for
each additional copy thereafter per order. | |
